Rarely, tbh. I often plan out all my characters, stories, settings, etc. Sort of like laying out a blueprint. One case that surprised me was one of the Highwaymen, Murdo Hope, suddenly caring for his casual kitchen boy lover (of legal age).
Have you ever been surprised by which of your characters hook up? Has a planned romance ever been derailed by chemistry that developed while drafting?

There is Fate but only of the inevitable (like death, doom, or deliverance). Most things flow on their own. The Future isn't written in stone, so changes can always happen. And Fate can be twisted (like a woman turning into a man to slay a demon)
In your setting is their fate or destiny at play? Or do things simply happen according to random chance. Is your main character a chosen one?
How do the people in the setting feel about the idea of fate and karma?

She dreamed of a rosebud in a calm river. The river got choppy & rough, turning red & white, then the rose blossomed.
She then woke up with cramps.

Definitely a combo of these. Bethany's family, herself, her husband & wife, all in Otherworld, where they would meet Jolly Father (the Otherworld version of Santa Claus), his wife (Mother Holly), and two kids (Snowflake & Kris Kringle).
