There's a company with 2.8 starts on 108 reviews on yelp

And 4.9 stars on 5,400 reviews on google - which all read very similar, and seem to concentrate heavily in the last several months.

The benefit of GDMT for HFrEF seems much greater / more important than of "GDMT" for HFpEF.

While the downsides--side effects, cost, pill burden, etc--are agnostic of benefit.

And yet I'm seeing a trend of "GDMT for HF" being increasingly conflated across the EF spectrum. Anyone else?

Interesting that they excluded uncomplicated cases.

Yes the complexity and risks of OPAT are greater in 6 rather than 2 weeks.

But much of it is front-loaded, e.g. PICC insertion, vanc levels/dosing, catheter thrombosis. Whereas cost of dalba linear with # of doses.

What the DOTS trial of Staph bacteremia tells us:
• Dalbavancin d1 and d8 vs daily IVs for WEEKS
• No left-sided IE/CNS/prosthetics
• Dalba not superior by DOOR endpoint, but similar clinical outcomes.
