You can't really offshore jobs involved in making it possible for 1-3 day shipping either (for the most part). The reason it is possible is because of many large warehouses all over the country and many many many drivers. Imagine if all these people were well paid.

(I forgot too that low income people on Medicare can get Medicaid to pay their premiums or just for coverage)

But it is probably because there was an uproar over poor people getting Medicare so they had to make a separate system. Plus, much less outrage cutting it compared to Medicare ...BUT!!

Thats not true at all. I have Medicaid in NY and my partner has Medicare. He has to pay for (at least) one separate private plan and he still pays co-pays and various other costs. Also, no dental or eyes. Meanwhile, I have full dental and eyes (so long as its not cosmetic) and pay $0 co-pays.
