Got a case now where they refused to give a pretrial detainee his insulin until he fell into diabetic ketoacidosis and died.
Every few months some agitated weirdo gets upset at me over the Atlantic post I did about how the prison system ROUTINELY engages in unimaginable abuse and lets people kill themselves, because they read it as “THERE’S NO WAY EPSTEIN WAS MURDERED”

okay I've had jobs where I worked with medical and dental insurance several times in my life, probably 5+ years at this point, and the horror I just felt reading this triggered a fight or flight response in my body

Well in my practical experience, what's been happening is exculpatory evidence gets caught on video, but the negligence happens to vanish, and spoliation is so hard to prove, there's not much to be done about it.

A hospital GC blaming plaintiffs and juries for verdicts that he calls not “sustainable,” without even a whisper of recognition that juries are finding facilities are committing life-altering malpractice (let alone commitment to change to prevent it from happening) is really something.
