conservatives don’t take issue with the abstract principles of what we offer as public services, they take issue with the “public” part where the whole point is integration of race/class/gender/“moral character” rather than laundering segregation through regional corporate chains
Rand Paul: "What is the Republican plan for healthcare? My plan would allow for people to buy their insurance through a collective. Basically to buy their insurance through Costco or Sam's Club or Amazon."

the bad writer journalist excerpts somehow aping the style of both raymond carver and rachel cusk (extremely derogatory). that’s how you write in 10th grade when your taste level far exceeds your abilities. all self-serious incantatory looseness with no substance

dreading 24 hour call. I will get through it but sometimes I feel silly for thinking in my 20s “I can’t just work some empty DESK job,” slogging through years of medical training and now be counting down until I can interview for a mostly-desk job

I'm kind of baffled how the "moderates" have apparently all decided to turn against climate policy at once, just as China reaches green electrostate liftoff mode. I thought these people were all about National Greatness and making Hard Choices

my dad was an early adopter of the Prius and hung out in online forums where middle-aged men quibbled about the speed at which to brake down ramps to maximize MPG, revealing Nerd Clout as the purest renewable energy source
Finally driving my first EV and can’t be the only middle aged dad now *obsessed* with regenerative braking and getting as many miles as humanly possible.

everyone in power folding on health insurance subsidies is so depressing. it’s obvious that the republicans are craven idiots who want to starve their supposed enemies in public science so HSA funds can go to antivaxx scams, but god, so many people and hospitals will go bankrupt for nothing

not to be a dick but I just saw an ad for TikTok on the MLB broadcast claiming the app is great for students and teachers. it showed a classroom of what looked like 18-year-olds was nodding at a chalkboard diagram of the FOIL method for algebra, which I regard as a bog standard 8th grade skill

I know the popular term for what AI supplies is “hallucination,” which isn’t the worst, but I wonder if other docs would agree that it’s more like the “confabulation” you get from an alcoholic/Korsakoff patient. there’s an eerie moral/perceptual emptiness in those words-with-the-shape-of-speech
This is something people who are chatbot-curious should be doing by default: ask it about something you *do know about*, not something you *don't know about*.
As an experiment, I asked ChatGPT to give me a walking route in my neighbourhood and to describe the scenery. I've also asked it to describe plot features of movies I've seen. It spews out bullshit.

made a good rich sauce to go over roasted veggies and sockeye salmon—miso, mustard, worcesterhire, soy, maple, a real kitchen sink of umami—and it mostly worked, but I’m finding that salmon clangs with big flavors. shoulda saved it for a chicken thigh

can’t help but think of politics through the lens of my recent time in “geriatric psychiatry” so that it seems like many aging politicians or voters are trying to make the economy into a memory unit where cheerful 1950s music is playing and the tvs show Andy Griffith 24/7
Interesting to note that the coal industry in the U.S. employs just a tad over 40,000 people, according to the BLS. The same source tells us that 64,000 people work at wineries.
Energy Secretary Chris Wright on Fox: "We're announcing today expanded programs to help the American coal industry. We're helping it because for years it has been under assault. It was out of fashion with the chardonnay set in San Francisco, Boulder, and NYC ... coal just makes the world go round."

I’m calling through complex hospital/medical phone trees and every single new recording says I should hang up and call 911 if this is an emergency. fortunately if I threaten to kill myself out of frustration I can consult on my own case

when I’ve worked with bad cases of somatic anxiety in the past one thing that kinda brings it all together is when we get to meet each other at the level of, no no it’s not that your body isn’t sick it’s that your sickness radar is sick, which is truly just as bad
I am lowkey writing an essay about how people hate when doctors diagnose them with anxiety despite the fact that anxiety is one of the most powerful illnesses of all time. It’s the only illness that can physically manifest itself. It’s the mental health equivalent of an autoimmune disorder

I’ve been stressed out recently from work and staying long every day to finish tasks (never enough) but one bright spot in my life has been reading. PGY2-3, I’ve been reading 50+ books a year, often before bed but even on overnight shifts. so much better than television! hugely rec the Kobo ereader
As you cancel streaming services, here is a casual reminder that only 16% of Americans read for pleasure anymore, and your local library has hundreds or thousands of books you haven't read.

They would love to see you stop by and renew your library card.

drifting through a sea of commentary on political violence, treading water because I have nothing original to say but also because these days the major aftermath of stochastic murder involves moneyed institutions disciplining their underlings to set an example. but I won’t be made to mourn a bad man

“harm reduction” has become such a widely disseminated and misunderstood phrase that non-healthcare people use it to describe the idea that “it’s okay for new things to be a little harmful.” before we get to the point where we’re troubleshooting clean vs dirty needles for heroin, don’t start heroin
