Just finished Jude Rossell's Midwatch. Wow! That is a middle grade book done well! So much fun. Grab it if you see it for your favourite 9-13 year old. Loved it.

Not an expert in the field but I suspect teachers and schools already KNOW which students are struggling with reading. What they need are solutions. Such as funding for libraries with librarians.
