Placeholder images in game dev are blatantly obvious so you don't accidentally keep them. Solid colors, "WIP" text, an out-of-place asset from your last game, etc.
When a company says that an AI asset got in as a placeholder, they're either mind-boggling incompetent or lying to you. Probably lying.
