What are prison releases in error so common?

In his statement today, Lammy explains some of difficulties he is trying to fix in the system.

Very complex sentencing rules.
Sentences worked out on paper.
Poor and ineffective systems (including emails).
Too few staff, who are stretched.

I'm not sure if others have noticed this, but interesting that one of the Prescott criticisms of the BBC is based on those of the 'History Reclaimed' group, which is also associated with the 'Restore Trust' group's attacks on & so far unsuccessful attempts to take control of the National Trust. 1/3

I suspect being sued for a billion dollars by Donald Trump, who is about as popular as cholera in Britain, for saying something true about what Trump did (albeit with a bad edit) will probably do the BBC's public trust and approval ratings quite a lot of good in the end.

The fact that the BBC has made serious culpable errors does not negate the point that there is a real and concerted right-wing media campaign to destroy it. Both points can be true at the same time and the campaign would not end even if the errors did.
