A concept that imo does not get the attention it deserves in critical care circles is the so-called “disability paradox”: that is, patients w/ severe neurological injuries often report satisfactory quality of life. This must be considered in goals of care discussions.

Weird but true: electroconvulsive therapy is sometimes used as a salvage therapy for super refractory status epilepticus (ie, seizures persist or recur after 24 hours of anesthetics). It’s all low quality evidence, mostly case series, but this is true of everything so far down the algorithm.

State psych hospitals all have these decaying composite tiles in the floor because they were built at the same time and never renovated. They break easily into sharp fragments and patients will use them for self harm, swallow them, etc. Somehow this keeps happening and nothing is ever done about it.
