"Lava River Cave" in Northern Arizona, USA, is a mile long.

It was formed ~700,000 years ago during a lava flow in the San Francisco Volcanic Field. As the lava river flowed, its exposed surface cooled and hardened. The molten lava beneath kept flowing, then draining away, leaving a hollow tube
