The highest-ranking officer in the room where Heffernan was killed by police, Staff Sergeant Lon Brewster, was let off without a disciplinary hearing because "too much time had passed" and he "wasn't involved in the alleged excessive force."

Hierarchy is only a system of reward, not responsibility.
