In Nintya, of the six forces, "unworld" was a corruptor. But apart from evil outcomes, arcanists considered it "dangerous but neutral"
In Nintya it was fairly common. An overcrowded city where most people lived in the sewer will do that.
In Bryznia there is enough hedge magic to go around that if something is not severe, you will recover.
In Vesperra, the new world still has surprise ailments, and snake oil doctors make them worse

I'll say it: I think its rly annoying and frankly amateurish that writers are constantly told their villains must be understandable or have sympathetic motivation to be interesting or realistic when all our big villains irl are in fact rich, stupid, selfish, trickster dillweeds and always have been
