Yeah, this seems like an issue of confounding by indication that is nearly impossible to solve (and certainly not possible to solve by adjusting for Gagne comorbidity score, age, sex, etc like the authors did). I have no doubt that hospice patients who require benzos and/or APs die sooner...

One more thing about working with large data in R, RStudio is notoriously memory inefficient, so if you have an R script that works for your purposes to generate the filtered/small dataset, just execute it from the command line and save the output to disk.

For pre-processing a shell script would probably be the best approach however be cautious if you're unfamiliar because ChatGPT has provided me with shell scripts on more than one occasion that would have wreaked serious havoc on my computer.

It is 100% correct that R is not the tool for this, but also 15 million rows is not thaaaat much and depending on your system RAM you should be able to do this in data.table. I routinely use data.table for datasets about that size because I refuse to use Python for statistics.
