I find the criticism somewhat misdirected - the typical EBM over nitpicking

OTOH albuminuria is *not* perfect

More valid criticisms?
- intensive BP lowering & dual RAS inhibition reduces albuminuria without reducing kidney failure

In a mouse model of IgA nephropathy, this #ASNJASN original research finds that FcγRIIB deficiency in macrophages and dendritic cells led to increased albuminuria, kidney dysfunction, and kidney injury. Read more: kidney.pub/JASN0835 #BasicResearch

RCT of dapagliflozin versus placebo.
Designed as crossover study. Looking at albuminuria.

Interestingly there were increases in the PO4, PTH, FGF23.

Thought to be due to increased absorption of PO4 by blocking SGLT2.

A persistent eGFR drop with SGLT2 inhibitors is usually a functional change, not CKD, unless markers of kidney damage (e.g., albuminuria) are present. Document as a medication effect, not CKD, unless other evidence suggests true kidney disease. Reassure and monitor the patient are best next steps

Although low albumin doesn't always mean more toxicity, and can be the opposite clinical effect. Ceftriaxone in cirrhotics is cleared more rapidly due to more free drug being available to clear, and loop diuretics require substantially higher doses in either hypoalbuminemia or severe albuminuria.

The study was 60 weeks.
Primary outcome was change in albuminuria
Secondary outcome was change in eGFR
They randomized 861 patients and 37 patients were switched from 3 mg to 24m mg after 9 months.

Joslin study found that early progressive renal decline (early GFR loss) begins in patients w/ norm renal function. It is the 1st manifestation of DKD (NOT albuminuria, as previously thought), said Dr Krolewski.
