It seemed like, for a long time, the NCAA policy, while imperfect, was a reasonable compromise. Athletes could take HRT for some amount of time (unclear if it was one or two years) to change divisions, and the result was that they were roughly as good after transitioning as they had been before.

This came up a few years ago in Connecticut, where the top two runners in some of the girls events at the state finals were trans girls (pre-HRT). I remember specifically that one of them went on HRT and fell back to the pack the next year, which would fit with what was the NCAA rule for years.
