Also notable that Mulder doesn't take the case as seriously as Scully does (he even suggests that the first victim faked her injuries) until a male orderly is killed.

The victim is flawed, too: she's combative, mean to the elderly patients in her care, and seen as a "problem" by her female manager. But none of that matters because even the "queen bitch" doesn't deserve assault, and Scully reinforces that point for the audience.

Gotcha gotcha. This piece has a strange combination of mythic/poetic exaggeration and what seems to be genuine military knowledge (see also the recipe for Greek fire randomly dropped in the middle of the story), and I couldn't tell on which side the bit about the softened helmet fell.
