What if someone you loved had CHF and didnt want to restrict fluids? Or someone with cancer didn't want surgery? I think I struggle with a moral imperative that sick people treat their illness in a way that makes other people feel better, or have a social obligation to do so.

Sure. Its easy when there is a fairly easy/available treatment for a relatively minor form of illness that usually resolves in weeks. How might this work with more severe illness with more complicated treatment, like chronic heart failure or cancers?

Take out the "mental" and see how it reads.
