Why? Low-effort physicians tend to have high-need patients. That might not be a uniquely Norwegian phenomenon.

Upside: ~everyone is weakly better off and regional disparities shrink

Downside: full information would still be far better

Should we increase effort by adding a choice of higher wage + lower salary?

Yes, if it attracts physicians who otherwise spend too little time with patients (large health effects of extra time)
