Especially as our trainees seem to spend less and less time in ICU settings overall, if we're not taking them to the bedside to teach them how to perform a neuro exam on a sedated patient or read the vent and drips... who is?

I really question the idea that there is even an inefficiency tradeoff to be justified here – how many times have I stood in the hallway watching a 20 minute conversation about what we'll do for every scenario of a hypothetical volume exam when we could have... examined the patient?

Since reading Bad Blood I have no chill about Elizabeth Holmes. She has in fact not been punished enough for falsifying lab results that were used in actual patient care.

What if sound alike errors were littered in your documentation but instead of "sounding alike" the substitutions were merely close together in a high dimensional latent space inaccessible to human understanding? Would that be good?
