IKR. The story is a little different for me in the details but painted from the same colour palette. It should never, ever happen again and yet society seems determined to ensure it does.

But even ignoring the classic catastrophic examples of code that "ran" until it didn't run right - THERAC, KCG, MCAS - what about code optimisation? What about documentation? There's a LOT that makes good code good beyond "it runs or it doesn't".
