Only time I’ve ever been in a car-bike collision in 40 years of heavy cycling was at a 4-way stop where we both ran our respective signs at full speed.

I don’t know about stop sign fatigue, but anecdotally I’m convinced compliance rates at all way stops are way lower. Drivers (and bikers like me if I’m honest) are less likely to do even a California roll if they know everyone else has to stop.

100% agree that urban FDs need to get over their love affair with monster trucks. Looking at your QT here, I’ll note that, for good or bad, the 20/26 foot widths are set so that one apparatus can pass another.
