Shared decision making for screening

Screening & risk assessment for health conditions may entail an array of benefits & harms. This overview of SDM uses cancer & CVD risk as examples of organized vs opportunistic screening.

Review highlights existing resources for patients considering health research participation; synthesises key principles for developers. Most tools target decisions re cancer clinical trials; future ones should address other clinical settings & study designs + emerging areas.

In 7, intervention targeted a specific clinical study; 11 covered trials generally. 8 print; 10 digital. 4 addressed cultural diversity. 13 cited theory/framework informing intervention. Interventions generally increased knowledge; little effect on participation.

We included studies (up to 2025) on intervention development/evaluation with data on acceptability/efficacy. Interventions aimed to improve decisions about research participation by enabling adult patients to address info needs or incorporate values into decision making.

An important albeit challenging aspect of conducting clinical research is achieving informed consent. We systematically reviewed the literature to characterise existing interventions designed to support communication & decision making about participating in health research.

OVERDETECTION EXAMPLE: Imagine a woman called Susan who develops a small, slow-growing breast cancer at age 75. The picture shows 2 possible scenarios that could happen to Susan... Susan's life span is the same, whether or not she has mammograms. So if she has mammograms, she experiences overdetection (a diagnosis and treatment she does not need).
