Others say it allowed the NHS to self-regulate and gave it more freedom.
NHS Scotland and wales are accountable to their respective parliaments and assemblies after it got devloved under Blair (I think).
Northern Ireland has their own system that also includes social care.

From what I know (and it isn't much), it's a company that gets all of the health spending and operates fairly autonomously. Some theorise it was a step towards privatisation as it effectively segmented running of england's health service under one non-government umbrella.

I'd assume because its a lot easier to detect a rogue character in a telephone number than an email.

A not insignificant amount of errors made filling in forms are due to accidentally hitting two keys at once.

Not in the day to day, but definitely had plot lines around a disease or an outbreak, and we've had players or well loved npcs catch them if unprotected.

Or if a player has picked some sort of cure, identify or treat disease skill; we'll put the odd encounter in there so they get to show that off.

That said, there have been games where I have dwelled on what happened or a loose end and that has caused a creeping sense of dread; but that tends to be as a result of the themes of the game and my brain running wild with "what if?".

Not really, at least not the game itself and not "in the moment."

I think theres too much of a disconnect; the closest I've got in the moment is tension and surprise, but not terror.

A lot of the time it depends on the style we want from a game. How survivable do we want it? How much combat do we feel like having? How much should be decided by dice rolls, and how much by player choice?

Though sometimes a system has a cool idea I want to explore, like Blades in the Dark.
