After a long end to summer, fall has arrived in Michigan. The lake temperature dropped from 68 to 58 degrees inside of a week. This was a wonderful swim that helps us acclimate for winter dipping. I didn’t wear my gloves but I missed them! #coldwaterswimming

I am an incorrigible hobbyist - I get really into things in phases. With a near endless supply of beach rocks, painting mandalas was a natural choice. Maybe I’ll take some to #KidneyWk to spread some joy.
I get nervous not biopsying because PLA2R results take so much longer to come back than pathology results.
Can’t wait for volume 2 of the postcards!

If you have patients with edema and #nephrotic syndrome, researchers at the University of Michigan would like to survey them about their symptoms. Patients are paid for their time.

Not so far. We just held off adjusting doses because the huge variability made no clinical sense. We have stable levels now with timing to GLP1a dose day, but what the weekly AUC actually is, I have no idea.
Anecdote, and a general nephrologist, but my patient’s tacro troughs were all over the place after starting GLP1a. They make sure now to draw the day before next weekly shot is due.
Going to need a new postcard book soon - going through them quite fast these days.

Practical issues very important as nephrology practices skew independent. I’d love to have a PharmD on my team but the reimbursement landscape makes that challenging/ impossible.
I think also if past experience has been that writing that Rx brings an avalanche of messages and calls about prior authorization or copays, it gets very easy to find a reason not to prescribe.
