4/ While light chain deposition disease can also lead to a nodular sclerosis pattern of injury, the immunofluorescence panel was negative in this case and no deposits were present by electron microscopy.
3/ Of note, in the absence of diabetes, nodular glomerulosclerosis can also be seen in what is known as idiopathic nodular glomerulosclerosis, thought to be secondary to hypertension and/or smoking (see references).
2/ Clinically, the patient carried a diagnosis of type II diabetes for >10 years with a recent hemoglobin A1c of 12%.
1/ The light mxicroscopic image of a PAS-stain demonstrates severe, nodular glomerulosclerosis compatible with nodular diabetic glomerulopathy, RPS class III.

This biopsy illustrates a rare example of glomerular involvement. Most emboli are identified “upstream” within interlobular and/or arcuate arteries. Prior to this biopsy, the patient had recently undergone an arterial catheterization procedure.

Cholesterol emboli involving the kidney may manifest clinically as acute kidney injury, hematuria, proteinuria (both non-nephrotic and nephrotic range), and even new onset hypertension.

Strong signal for better kidney biopsy adequacy favoring 16G needle over 18G (w/ = complication rate) in pediatric pts 12-18 years-old, extending established norm 4 adults. Only 4 children <12, a smaller 18G needle is safer. #KidneyBiopsyAcademy @kidneycon.bsky.social @arkanalabs.bsky.social
3/ By electron microscopy this case demonstrated numerous, randomly arranged fibrils that measured ~10 nm characteristic of amyloid fibrils within mesangial areas with extension into numerous capillary loops. Congo red stain was positive in glomeruli.
2/ This finding is classically seen with amyloidosis. Other glomeruli in this case demonstrated areas of PAS-weak to negative staining of mesangial areas which is another feature that should raise suspicion for possible amyloidosis.
1/ The light microscopic image of a Jones silver stain shows a glomerulus with demonstrates numerous elongated, fine spicules which some regard as the "eyelash sign".

However, the prominent IgG deposits also involving the associated arteriole in this case are an important clue that helps argue for a secondary etiology. This patient, in fact, was found to have systemic lupus erythematosus. (2/2)
