I think protein supplementation might become even more important with glipination, ideally in an easy to consume form because otherwise the risk of not consuming enough may increase fast. Hopefully an area that will get some research attention

Up next! ☀️📚 We're going back to the guidelines with KDIGO IgA, which is already out of date (isn’t that awesome) #NephJC

I’ve been suspicious of Cochrane SR/MAs ever since the attack dog name calling incident against Dr. @hswapnil.medsky.social circa 2018…. they were mean & wrong, #nephjc

I’m not even sure the current data actually qualifies for a meta analysis?! We definitely need better quality studies and more patient focused outcomes #NephJC
just higher vs. lower plant-based diet…. but it’s ok too many limitations with the analysis, so the conclusion is not reliable, #nephjc
yes, patient engagement with collaborating patient partners is important. A requirement for many funding applications too, #nephjc

#NephJC - yes… it is a failure of nephrology and dialysis that there are so few QOL studies or any studies with patient-relevant and clinically relevant outcomes. This highlights the importance of taking the time and effort to engage stakeholders when developing research questions

17 trials with 955 patients
DAPA-CKD had 4,300 CKD patients one of half dozen CKD #Flozin trial

This is so under studied it is pathetic.

No one should be on low protein diet.

It doesn’t work! People who recommend it (such as KDOQI) do not know how to read and interpret studies

Time to move on

According to international recommendations, 0.8 g/kg/ day is fit to cover the needs of 97% of the population. 0.66 g—it's the daily average accepted intake #nephjc

Given the lack of evidence supporting dietary protein restriction (and potential ⬆️mortality), the 2024 @kdigo.org guidelines currently recommend maintaining a protein intake of 0.8 g/kg body weight per day in adults with CKD G3–G5 (level 2C). #NephJC

Trials examining clinical outcomes in patients with CKD, don’t support any particular threshold of dietary protein. Meta-analyses have demonstrated NO difference in mortality, progression of CKD, or onset to dialysis with low protein vs normal protein diets. #NephJC
