What Post Exertional Malaise #PEM actually means - the delayed worsening of symptoms after exceeding a pathologically reduced energy threshold -
is now reframed as simple “fatigue after exertion” or a “drop in performance.”

The “Post” is stripped from PEM - and the pathophysiology along with it.

I was just reminded of this old video on how I think medicine's partitioning of the human body into 11 discrete organ systems is anachronistic and contributes to misunderstanding of pathophysiology & misdiagnosis of diseases that fall into the cracks between systems.

But anatomy and physiology is so weird and fun. it’s wild to know some of the basics (as an EMT, your understanding of physiology and pathophysiology is decent, but verrrry practical and basic, on a systemic level). but going into the cellular and biochem level feels like entering a new universe

I have to by law supervise highly seasoned third year residents who are functionally attendings and can explain complex pathophysiology to patients in clear language, and this guy is unsupervised practicing #medicine on millions of people
