Treatment protocols for glial brain tumors have remained largely unchanged over the past decades, with neurosurgery serving as the cornerstone of initial treatment, followed by radio- and pharmacotherapy.
In neurosurgery, technological advances like intraoperative MR imaging (ioMRI) have made it possible to push the boundaries and maximize the extent of resection, which is linked to improved patient prognosis and survival.

The study compared the extent of resection in glioblastoma patients with and without ioMRI, and the association of ioMRI with the incidence of adverse events.
A total of 406 patients with histologically confirmed WHO grade 4 glioblastoma tumors were retrospectively included; 74 patients were undergoing clinically and surgically indicated ioMRI.

The study shows that in this patient cohort, the use of ioMRI led to substantially enhanced rates of gross total resection without an increased risk of postoperative adverse events.

Gliomas are the most common primary intracranial tumors in adults.
Achieving the maximal extent of resection has been shown to enhance both progression-free survival and overall survival in the surgical treatment of gliomas.
The optimal surgical strategy for gliomas requires balancing aggressive tumor removal with minimizing the risk of new or worsened neurological deficits to ensure better outcomes.

Given the diffuse nature of gliomas, neurosurgeons face significant challenges in differentiating tumor tissue from normal brain matter, making the selection of optimal imaging techniques crucial for precise resection. Since low-grade gliomas usually do not exhibit visible fluorescence with 5-ALA, intraoperative MR imaging (ioMRI) remains a useful tool for imaging throughout surgery,
as well as for enhancing functional navigation when awake surgery is not an option.

In practice, the significant benefit of ioMRI technology becomes particularly evident with complex, large, and eloquent gliomas, as well as with small or minimally invasive approaches, such as the supraorbital keyhole approach, where the anatomical overview is limited.

In the case presented here, ioMRI was recommended by objective preoperative radiomics analysis and proved helpful in increasing the resection rate from 95% to a gross total resection (100%).

However, when considering ioMRI, additional clinical and surgical routine aspects must be addressed. Prolonged anesthesia increases surgical risks, and the extensive operating room infrastructure and expertise requirements challenge smaller clinics. Thus, while we believe in the value of ioMRI in glioma surgery, radiomics analysis is a promising tool for determining which patients stand to benefit the most.
