super sick patients who should mount a lactic acidosis but dont often have autonomic dysfunction with inability to produce endogenous epinephrine; they may respond well to exogenous epinephrine

Not hugely practice-changing, as many folks are already using bicarb to reduce dialysis risk based on BICAR-ICU-1

Lack of mortality impact is unsurprising given how difficult it is to prove in ICU (discussed: emcrit.org/pulmcrit/mor...)

Reduced dialysis & bacteremia are important pt-centered wins

Wie oft Leukos bei dicker Sepsis bei Aufnahme noch (hoch)normal sind, und dann am nächsten Morgen 8 Stunden später bei 20 tausend.

Ich glaube wir nutzen Diff-BB und NLR zu wenig.

NLR isn't perfect, but it's a whole lot better than WBC

it can be an early signal of serious trouble

or, if NLR is normal, it argues against sepsis (look for some other problem)

NLR-blind clinicians will think you're a genius 😆

but, understanding the physiology of how to convert a VBG to an ABG is a more generalizable and powerful approach to this problem

This is a great way to use LLMs in medicine. Simply letting the LLM do all the work will invariably lead to numerous issues but a "triple-threat" approach makes a lot of sense

glad to see validation for using balanced crystalloid in DKA >10 years after I started pushing this (emcrit.org/pulmcrit/fou...)

But balanced crystalloid in DKA is so passe now

The next frontier is using KAcetate & KCitrate to avoid NAGMA

c'mon... let's get that published by 2035 😁

Are you using 0.9% NaCl or balanced salt solutions such as Lactated Ringer's or Plasmalyte to resuscitate your patients with DKA? This paper suggests that we should be using the latter. 🎩 tip to the authors.
