In practice robust error handling at both the instrumentation and app layer handles most cases, but this is also an issue with OpenTelemetry’s tracing design. You could theoretically have a request fail before closing any spans at all and somehow evade the error handling

of course you should have automatic instrumentation of RPC calls, that's table stakes and part of @opentelemetry's promise -- to bake it into every HTTP, RPC, or service mesh library and framework.

that's just bones. a skeleton falls over. you need muscles and ligaments to walk.
