There is a reason why Fischer, after waking up at the end, doesn't have a "I had the craziest dream, and each one of you were in it. Nothing sus about that" moment. He was really another extractor who played his part in this "therapy session". They were all in it together to help Cobb.

The entire movie is a dream. Cobb kept himself in permanent dream state because of his guilt over Mal. Ariadne was really a therapist hired by Miles. The titular "inception" was her planting the idea that would allow Cobb to finally let go of that guilt. I'll take no further questions at this time.
