@mcuban.bsky.social 4. Isn't a catch. It's the sensible way to run a healthcare system. High quality PC physicians should be guiding care and advocating for patients. You need to cut costs from your grossly inefficient system with its over servicing. PC physicians are your best bet to do that.

Hanif Esmail is doing the appropriate trial in CXR +ve but sputum -ve. (RADIO-TB). You have identified as related but different phenotype of CXR -ve and sputum +ve, which requires a similar study IMO.

Fascinating- great study. So sputum testing seems necessary for HHCs. Key question for a clinician is- what is appropriate therapy for a HHC with no symptoms, a negative CXR but incident TB? My bet would be 2 to 3 months is plenty, but we need a trial.

Was discussing the public health approach to HBV that is required in Africa. There is lots to learn from the public health approach to HIV. The concept that disease is chronic and requires chronic medication doesn't come easy to people who are used to 'curative' treatments.
