collisions are rare nowadays here but afaik all the recent train on trains ones are due to a driver passing a signal at danger - theres some devices to reduce the chance but theyre not 100% foolproof. one recent one was due to the brakes not working properly due to leaves on the line cause slip

there's a thing called a "track circuit" that detects when a train is in a signal section and it sets the signal before to red and the signal before that to yellow (which means slow down to prepare to stop on a red). so human error on signalling cant cause collisions IF drivers follow signals

this system is just the scheduling and train allocation bit. signalling software is totally different and disconnected, humans allocate paths using software that looks kind of like this (stolen from the simulator SimSig) but there are mechanical interlocks that change signals to red/yellow
