I’d love this to be normal practice - I’ve always said everyone should have a TEP form in hospital because it normalises these discussions. Early versions where full escalation appropriate, with gradual change to comfort as priority

Absolutely agree.
No-one with a LTC should present in a foreseeable crisis without a plan for action. Useful to guide patient & family, carers, first responders, ED etc, & plans made collaboratively respect the patient's values.
