When selecting scales, researchers seem to care about what others have used, rather than underlying construct validity. Putative is what gets them published.

Vague hope in research doesn't help people who are struggling to live right now. We need more care (especially financially) for people right now not just vague hope in some sort of treatment in the future.

I'd give it years. Researchers have repeatedly mischaracterised or oversimplified PEM for many years, so it is worth taking time.

I would use a multi phase approach trying different qualitative research methods and asking participants on an ongoing basis whether the conclusions are reasonable or not. All of that before I'd even start to decide whether a survey/scale is even capable of capturing the range of experiences.

I'd suggest they start with qualitative interviews of patients around the world (from a variety of backgrounds) to start with to learn about PEM experiences in-depth, before writing any sort of scale/survey questions.

I'm sorry but I strongly disliked the questionnaire and think it makes numerous incorrect conceptual mistakes. I find it upsetting that researchers keep skipping the most important step - qualitative research eg actually talking to patients.

An incredible second day too, I just hope this doesn't give any more ammo to the "4 day test" crowd. Some good bowling/bad batting in one test doesn't justify going against the tradition of 5 days.

Why do most neurologists have so little understanding or desire to understand the neurology and relationship between fatigue sensation and central fatigue (and why it exists)?

I like how they call it a Randomised clinical trial and not a Randomised Controlled trial as many rehabilitation studies pretend to be, but are not. I thought the tDCS-sham placebo was novel.

This is a blanket ban on bikes and scooters, most people are incapable of riding at that speed without falling over. And yes, joggers/runners and some walkers all exceed that speed.

Pehaps two effects, 1st is people who are just not used to exercising *at all*, 2nd is early onset of hyperventilation can be due to excessive muscle afferent feedback (who may already be suffering PEM). Note that *ALL* participants will functionally hyperventilate towards the end of a maximal CPET.

Other 2 day CPET studies haven't really noticed such patterns (VE/CO2 ratio) in most patients there have been a lot of them so there may have been something specific about this study eg. the sample group or the mouth piece as mentioned.

It bothers me that the authors demonstrated so little understanding of exercise physiology in the manuscript, and peripheral factors that can lead to altered ventilation.

It's delusional if he thinks the market employment model is willing to support disabled people in any way. They discriminate and then lie, to skirt any anti-discrimination laws because it's all performative, they don't care about disabled people, only saving money regardless of harm.
