It would be great to have 2B6 isoenzyme as a part of any P-450 panels to determine polymorphisms (S-isomer pathway and risk for cardiotoxicity). This would enhance the safe use of the methadone in HAPC.

Hopefully, with affordable genomic panels that will be covered by insurance. It would certainly reduce a lot of therapeutic blind alleys and the need for "time-limited" trials to determine efficacy/tolerability.

Where are his hearing aids? With the benefits they all receive, would you think he would have the top of the line. Not to mention that hearing loss is a modifiable risk factor for dementia!

Ok, just be careful how you schedule the Q6 so your patient isn’t receiving at 0200 each night. Interrupting sleep for meds = risk for inpatient delirium.
