It's a bit of a mix, the music is emulated in real time using NukedOPL3, but the sound effects are pre-rendered - recordings created by running a modified version of the original DOS game in DosBox Staging, to be exact. Not possible, at least not with the original DOS engine. The original game already struggles with the 640k memory limit, and episode 4 has larger levels with higher object counts, there are way more and much larger sprites, etc. So it could be done in theory but would be a massive effort (cont.)
