If it's fast enough, you could have the problem (one of several) we had with the main elevator in Prey. You'd jump off of the surface as it was going up, then it would hit you and kill you with "falling" damage.

The characters were modular so we could vary up the combinations. Maybe a bug where that part didn't load? I don't remember doing any intentional missing head vignettes.
