In 2024/25, the specialist homelessness teams:

✅ Helped 4,778 patients experiencing homelessness, a 41% increase on 23/24
✅ Reduced returns to rough sleeping by 62% and to sofa surfing by 33%
✅ Registered 70% of patients with no prior access to primary care with accessible GPs

The team’s success highlights the critical role the health system can play in preventing and ending homelessness - making a transformative difference in the lives of people facing homelessness.

With funding re-established, we’re thrilled that HIT’s life changing work is continuing.

Prioritising safe hospital discharge, the multidisciplinary team's highly skilled, person-centred approach has reduced the number of patients returning to rough sleeping by 60%.
