The first three things on your list! A beautiful cover will grab my attention, a great pitch will make me crack open the cover, and a strong opening chapter (or, more likely, a strong first couple pages) will seal the deal.

It really depends on the individual text! Some need 2-3 rounds of editing and re-drafting, and some need 5-6. The best way to figure out if your book is publication ready is to run it by beta readers and/or a professional editor (the latter can even help reduce the amount of drafts necessary!).
