Good analysis, as always, though I’m not sure the administration’s aims are noble or admirable. they are cynical. This is not a proposal for peace but for surrender.

Copenhagen is different, politically, from the rest of Denmark though. The prime minister has been a social democrat since 2019 and there is broad political and voter consensus on a strict immigration policy for more than a decade + far right parties are mostly marginalised here

It would be better to find societal solutions to societal problems, sure. But as a practicing clinician this issue is a non-starter. We have to help patients within the existing framework. And Ozempic is great at that

BMI requirements vary quite a bit between centers Denmark is generally pretty strict - we usually require BMI < 30 at my center.

I think everyone is rightly sceptical if the large effect size, but given the high risk CVD risk and low cost/risk with fish oil, I feel it is too reactionary to just say “we need more trials”. We always need more trials, but they’re not likely to be forthcoming any time soon.

I suppose we would hypothesize cardiovascular protection with fish oil… but those HR’s are really something. and the separation is within the first 6 months. I’m certainly also sceptical, but I still feel like we should start perscribing.

I just think that a trial with a composite outcome would give us much better information and would not be unreasonable to demand from a big company like Bayer..

Fair point! It’s just that when they need 33000 patients to power a trial for hard outcomes, as it was for CONFIDENCE, I get a little worried whether there is an relevant benefit

I am willing to accept surrogate outcomes for rare diseases, but for things like DKD there is really no good reason to not demand hard clinical outcomes IMO
