This is a good article but I want to note that the page q_dork.itch.io/consume-me was never indexed since its 2018 publishing because it didn't meet minimum indexing requirements of having an uploaded file. It was not affected by the NSFW audit

The review process doesn't involve you. Your page would have been put into a queue for someone to look at, they would see that it's not spam/scam/abuse, then click OK, and it would be eligible for indexing.
I don't see any evidence of any of your pages being put in the review queue.

Both. We use automated heuristics to flag some pages for review before indexing, but generally speaking pages can be published without any explicit approval.
Too many pages are published per day to review everything, so we try to prioritize what we do look at

Each participant must go to the bundle's URL to approve the bundle. No notifications are generated to prevent spam. If you go to the bundle's URL on your other account you will be able to approve the bundle.

I'll take a look, did you recently update or change your tax information on your account. That's the most common issue with a payout not being eligible to be sent.

I actually went back to pgbouncer shortly after. pgcat was introducing a lot of per-query latency at the load level of the machine. I ended up provisioning a larger machine. We're still debugging some performance issues though so may experiment with pgcat again.
