Most patients included in this IPD-MA were treated with either Dialectical Behaviour Therapy (DBT), Mentalization Based Therapy (MBT) or a digital intervention based on Schema Therapy (priovi). The treatment effect was not affected by whether the treatment was online or in person.

Psychotherapy seemed to be less effective for individuals who also suffered from depression or reported sexual abuse.

This IPD-MA looked at numerous patient characteristics and found that borderline patients with comorbid anxiety disorder or comorbid substance use disorder derive the greatest benefit from psychotherapy.

IPD-MA are better than conventional meta-analyses at identifying which patient groups benefit most from a given treatment.

Oder in Alltagssprache gesprochen: wenn diese widrigen Kindheitserfahrungen sehr im Mittelpunkt des Erlebens stehen, dann sind möglicherweise auch psychische Probleme stärker ausgeprägt.

Zusammen mit ausgeprägten Schwierigkeiten, belastende Gedanken und Gefühle loszuwerden, hängt diese Art des Selbstbildes mit der Schwere einer Reihe von psychischen Problemen zusammen, unter anderem mit Depressivität und Ängstlichkeit.

Das Ergebnis: Veränderungen des Selbstbild („Ich sollte nicht so fühlen und denken, wie ich das tue“) hängen besonders stark mit widrigen Kindheitserfahrungen wie emotionalen Missbrauch zusammen (als beispielsweise der Erfahrung, als dumm, faul oder hässlich bezeichnet zu werden).

Eine Möglichkeit, der Antwort näher zu kommen ist, die Kindheitserfahrungen, die Schwere der Symptome und mögliche Prozesse, die zwischen beidem vermitteln, als Netzwerk zu betrachten. Dann bekommt man rasch einen Überblick, wie diese Dinge zusammenhängen könnten.

A number of studies have compared digital interventions to CBT. Both are about equally effective, meta-analyses suggest. But obviously, we also have to consider preferences: digital interventions are less effective in individuals who prefer f2f-therapy. References: bit.ly/DiGA_2024

A recent meta-analysis by Felix Butt from Stephan Köhler`s research group at Charité, Berlin, suggests that unguided digital interventions have effects both quality of life might and on depression (g = 0.29 / 0.37).

Many researchers as well as clinicians and their patients would argue that an evidence-based intervention should show effects both on symptoms and on functional outcomes to receive a strong recommendation for use.
