The game the anime is based off is pretty bloody and horny (and most characters are minors). I haven't played it myself, but I saw my friend play it three years ago. In the game it might not be so graphic since most of the gameplay is like a pixel 2D RPG with image stills for cutscenes.

Yeah unless you are on JP datacenter, you would have problems with NieR raids. Japan seems to do them more often than anything else aside from Crystal Tower and XI raids.
But even then raid 3 is uncommon to see in JP on a normal day lol.
