Especially the latter. The red herring of sepsis may expose patients to diagnostic delays, perhaps increasing their odds of a worse outcome. Not sure if this has been looked at properly.

When it comes to sepsis, we seem to be fixating on the left column (TP and FN), at the expense of the FPs.

On an unrelated note, I asked ChatGPT to help me interpret the P value in that non-inferiority statement.

'[...] at 5.4% (19.4% [170/878] of pts receiving std care; 20.9% [184/879], PCT; abs diff, 1.57; 95% CI, −2.18 to 5.32; P = .02)'

Can someone smarter than us (GPT & me) help confirm or refute?

I have … thoughts. Not sure what the study was actually assessing - odd primary endpoint given intervention. I wonder how robust the sample size assumptions (6% ARR) were.

But mostly just goes to show what we do peri-operatively has such a marginal effect, if any.
