skunks are not a #rabies virus reservoir in Idaho, bat-maintained rabies virus variants, including silver-haired bat variant, are enzootic in the state.
likely 3-step transmission chain: rabid silver-haired bat infected a skunk, which infected donor & led to infection of transplant recipient

#Rabies risk varies by species, geography, & exposure circumstances. Rabies virus is maintained by specific animal reservoir species in continental USA (i.e., bats, raccoons, skunks, and foxes); however, any mammal can be infected, and approximately 800 cross-species infections were reported in 2022

5 weeks post-skunk scratch: donor was confused, had difficulty swallowing/walking, hallucinations, and a stiff neck. 2d later unresponsive: hospitalized: never regained consciousness & declared brain dead/removed from life support on hospital day 5.

Per family: a skunk approached the donor as he held a kitten in an outbuilding on his rural property. The donor sustained a shin scratch that bled, but he did not think he had been bitten. According to the family, the donor attributed the skunk’s behavior to predatory aggression toward the kitten.

#penumbra sign has a high specificity of 96% but low sensitivity of 27% for musculoskeletal infections & helpful in differentiating neoplasm from infection. The proximal tibia is most frequent site of involvement & Staphylococcus aureus is the most commonly identified pathogen

#Penumbra sign: characteristic MRI feature of subacute osteomyelitis but can be seen in tuberculosis osteitis, osteoid osteoma, chondrosarcoma, eosinophilic granuloma as well as in benign cystic neoplasm and intraosseous ganglion

-High risk (contact w/ livestock closed spaces) abattoir workers, vet cattle necropsies foremen & milkers
-Medium risk (contact w/ livestock open spaces) tractor op, herders, feeders, vet & living in cowshed
-low risk (no direct w/ livestock)cowshed owners,clerks #bovis #TB

Widespread milk pasteurisation and strict hygiene measures in livestock management have gradually reduced transmission of bovine tuberculosis in developed countries #bovis

#Mycobacterium #bovis responsible for zoonotic #TB. In humans: occurs following consumption of unpasteurised dairy products. However, transmission via occupational exposure may occur via inhalation of aerosols exhaled by infected animals or humans & from direct contacts w/wound
