It really is an unpleasant thing to deal with and so sorry this didn't work for you. Some of the other suggestions might help and hopefully a vet can help, at least to rule out anything physical. I hope you can resolve it for both of you.

We had a problem with this and found that it was due to scent marking. If you are told or think it could be that we found cleaning with diluted bio clothes washing liquid and then spraying with "wee-away" worked. I am in UK but I'm sure this product will be fairly universal.
