It comes and goes really. I had too many items, then had none, then too many again. I wasnt a fan in general of the combat so it just being harder wouldnt be a good thing to me.

For combat, no, I think the game would be worse with higher difficulty combat. Puzzles, probably. I played on "hard" or whatever the one up from that is and they were just fine
