I think I'm getting closer. I found another reference to the NIH announcement in 1999: a Science article (by Greenberg et al.) "comes in the wake of the NIH announcement to make biofilm research a priority." [link to newswise press release]

Rapid infection control will only work if it is targeted on how transmission arises.

It says either HCWs are not following guidance or the guidance is ineffective at preventing transmission and infections.... I am for the latter.

This is a single celled fungi. It has no ability to cause outbreaks - its not a general with complex battle plans ready to invade. IT IS US who provide the perfect niches - the ICU is an agar plate promotes transmission and enables proliferation on invasive devices and various liquids...
