Most of the time you don't need cutting edge, you only need cheaper doctor visits to do proper preventive medicine. Costa Rica have a good, and quite cheap, healthcare system due mostly because it does preventive medicine.

I think is clear that when you want HRT and the physician suggest therapy, that the physician doesn't think you're trans.
Because therapy doesn't alleviate gender dysphoria.
